NarrativeAt 10:30 a.m. on November 16, 2017, an employee released the straps from the load and climbed up on the back of the flatbed. A forklift was ready to unload the steel bar when the employee's feet became tangled in the straps causing him to fall off the truck and land on the ground on the passenger side of the vehicle. Simultaneously the steel bar had momentum and continued to roll and fell on employee killing him instantly.

NarrativeAt 12:00 p.m. on June 27, 2017, Employee #1 was working from a step stool to scrape popcorn from a ceiling. During work, the employee slipped and fell, striking a nearby vanity with his torso. The employee sustained fractures to his ribs, which he was hospitalized and received treatment.

NarrativeAt 11:50 p.m. on March 3, 2017, an employee was working to remove a roller board from a metal hook that it was hanging on. As the employee tugged on the roller board, it suddenly released from the hook and caused her to become unbalanced. The employee fell backwards, striking a linoleum floor below with her left hip. She sustained a hip fracture, which she was hospitalized and received treatment.
