NarrativeAt 11:00 a.m. on September 28, 2018, an employee was installing french drain pipe in 12 foot deep trench and was caught in the trench when the walls caved in and buried him. The employee was killed due to asphyxiation.

NarrativeAt 3:00 p.m. on November 2, 2017, an employee was cleaning a roof to patch several holes. The employee fell approximately 11 feet from the top of the ladder and suffered a broken left tibia and displaced her hip from the ball joint. The employee was hospitalized.
