NarrativeAt 1:30 p.m. on September 6, 2017, Employee #1, the Special Effects Coordinator, was setting up a test run for a shooting scene in "The Last Ship", involving a metal sheet and squib. A piece of metal came off of the metal sheet and struck Employee #1 in the cheek. Employee #1 sustained a puncture wound to his cheek and was hospitalized for his injury.
On Tuesday, March 13th 2012 OSHA reps took a report of a serious accident in Valencia, CA which had occured previously at Sonic Electronix Inc.. A Personnel had been injured when the employee's finger was amputation by router

NarrativeAt approximately 11:00 a.m. on February 23, 2012, Employee #1, with Sonic Electronix, Inc. (an electronic retail and car stereo accessory installation service), was cutting a wood form for a speaker box. While using a router table (Bosch Model Number 1617EVSPK), his left middle finger was amputated. At the time of incident, the router cutting bit was not guarded.
